The Organisational Questionnaire

The CAREFUL Organisational Questionnaire attempts to quantify to what degree your organisation - be this a team, or an entire hospital - measures-up to the CAREFUL ideal.

The Questionnaire consists of 28 questions - one for each element of the programme. By consensus, you and your fellow leaders or staff-members are invited to score your organisation out of 5 for each question.

The average score gives you your "CAREFUL Score" - a value which will rise if you implement the CAREFUL programme effectively. The questions are worded so that they give you an idea of how to improve the score.

This means that the questionnaire not only gives you a current value - it also provides an approximate road map. Taken in conjunction with the documentation for each element, your answers to the Organisational Questionnaire give you an idea on how to improve your organisation.
